films that were actually intended as "real" films, and not structuralist experiments or something:

Testament of Orpheus (1959) was pretty baffling. it's part of a triology of films, the first of which was a surrealist silent film from the early-30's, and the second of which was a mostly-straightforward adaptation of the orpheus narrative.

page of madness (the japanese silent surreal horror film from the 20's)

hard to be a god (mentioned above in its russian name) took the sci-fi novel it was based on and turned it into a drowsy, muddy medieval nightmare

things, the late-80's canadian horror film

in terms of bizarre aesthetics/setting, the first max headroom episode seemed continuously weird. it was some bombed-out vision of the future with aging 80's punks wandering around a very grim landscape somewhere in england.
